Apostille3 EASY STEPS. Some notarizations require the Notary to put the signer under an oath, declaring under penalty of perjury that the information contained in a document is true and correct.

Step 1.  Notary provides you the Department of State’s Apostille and Notarial Certificate Request Form to complete.

Step 2.  Notary determines which documents needs to be notarized with a Jurat Statement or Certification by Document Custodian.
